A Care Coordinator looks at all four.
A health-related social need, such as food, housing, or transportation.
An eligible Medicaid Managed Care plan.
At least one Enhanced Population.
Any other rules for the service, such as medical need, paperwork, approval, and whether another program already covers it.
Who the program may serve
You must be in at least one of these groups. A Care Coordinator still checks the other rules for each service.
Medicaid high utilizers.
People enrolled in a New York State-designated Health Home.
People with substance use disorder.
People with serious mental illness.
Adults or children with an intellectual or developmental disability.
Pregnant people and people up to 12 months postpartum.
Certain people up to 90 days after release from incarceration who have a chronic condition.
High-risk children under age 18.
Examples
Rent and temporary housing
Up to 6 months
Some Medicaid Managed Care members with an unmet housing need may be authorized for rent, back-rent, or temporary-housing support for up to six months. You must meet the other program rules. Payment goes to an approved landlord or housing provider, not directly to the member. A Care Coordinator confirms approval, amount, length, and availability.
Pregnancy and postpartum nutrition
Up to 12 months postpartum
Some pregnant or postpartum Medicaid Managed Care members with an unmet nutrition need may be authorized for meals, food prescriptions, or pantry-stocking support during pregnancy and for up to 12 months postpartum. Some meal services may provide up to three meals per day, seven days per week. A Care Coordinator confirms the service, length, and availability.
