People with Medicaid Fee-for-Service and Medicaid Managed Care members who do not qualify for Enhanced Services can get help finding existing federal, state, local, and community programs. Eligible Medicaid Managed Care members may qualify for Enhanced Services if they have an unmet need and meet the other rules.

Navigation helps people find programs. Enhanced care management provides social-care support, not medical care.

    Navigation Services (Eligibility Assessment and Navigation)

    People with Medicaid Fee-for-Service (regular Medicaid) and Medicaid Managed Care members who do not qualify for Enhanced Services can get help finding existing federal, state, local, and community programs. This does not include Enhanced Services.

  2. 1.2a

    Eligibility Assessment

    For eligible Medicaid Managed Care members with an unmet need, a Care Coordinator checks the Medicaid plan, Enhanced Population, and the rules for each service.

  3. 1.2b

    Enhanced HRSN Care Management

    Eligible Medicaid Managed Care members who meet the other rules may get help with referrals, planning, education, and follow-up. This is social-care support, not medical care.
