The screening does not approve rent, repairs, equipment, or other housing services.

    Home Accessibility and Safety Modifications

    Changes to the home may help an eligible member move around and live more safely.

  2. 2.2a

    Mold and Pest Remediation

    Eligible members may get help treating mold or pest problems in the home.

  3. 2.2b

    Ventilation Improving Systems

    Eligible members may get ventilation improvements for healthier air in the home.

    Equipment Provision

    Eligible equipment may include a medically necessary mini-refrigerator for safe storage.

    Asthma Remediation

    Eligible members may get help fixing home conditions that make asthma worse.

    Recuperative Care (Medical Respite)

    Eligible members may get a safe, short-term place to recover after medical care.

  7. 2.5

    Rent / Temporary Housing (Rental Subsidy and Back Rent)

    Eligible members may get up to six months of rent, back-rent, or temporary-housing support. Payment goes to an approved landlord or housing provider, not to the member.

    Utility Setup / Assistance

    Eligible members receiving rent or temporary-housing support may also get help starting utilities, paying eligible past bills, or covering up to six months of utility costs. Payment goes to the utility company.

    Pre-Tenancy Services

    This may include help with housing options, documents, applications, and tenant responsibilities.

    Community Transitional Supports

    Eligible members who found a home may get one-time help moving in and setting it up.

    Eligible members may get help understanding a lease and keeping stable housing.

    Eligible members may get help finding housing, planning a move, and completing next steps.
